The place is very clean, professional and welcoming. From the first time I stopped to make and appointment till my service was completed I was met by friendly, helpful and caring staff. This attitude by staff made the procedure less threatening. The follow up procedure was incredible. The assistant was very warm, compassionate and explained everything I needed to do to look after myself. If anything appeared amiss to call immediately for help. I was called in the evening by the Doctor to see how I was and if I had any issues, how refreshing to have that happen. I was contacted the following 2 days to see how I was doing, if I had any issues or concerns. This was so greatly appreciated and a service 1 rarely gets in this age. Normally once ur out the door ur on ur own. But totally different here. A service that was grateful appreciated. Would I recommend coming here u bet without hesitation.

Okay I hate needles, procedures and just anything that isn’t the norm. I told the surgeon my concern and right away he was able to put me at ease by walking me through what he was doing. It was so quick, so painless and stress free I walked out of there wondering if I should book something else with them! Long story but worth the time to write this review. Highly recommend this office!

I entered the room with the assistant with some trepidation, got settled and then prepared for the pain (physical and emotional) of the extraction. I immediately felt at ease as soon as the assistant started chatting. Between her and Dr Bahi, the time went very quickly and painlessly with their banter. I received thorough instructions of the care of the site and I was on my way. I followed the instruction and had an easy night. I received a follow-up call the next morning to see how I was and if I had any questions. Thank you Dr Bahi and team

Understanding Bone Grafting

In a dental context, bone grafting is a surgical procedure that rebuilds and fortifies the jawbone. It involves placing new bone or a bone-like material into areas where bone has been lost. Bone loss in the jaw is a common issue that can result from a number of factors, including:

  • Periodontal Disease: Chronic gum infections can destroy the bone supporting the teeth.
  • Tooth Loss: When a tooth is extracted, the lack of stimulation from chewing causes the underlying bone to resorb, or shrink.
  • Trauma: Injuries to the jaw can lead to fractures and significant bone damage.

A healthy, dense jawbone is critical for dental implants. The implant post relies on a process called osseointegration, where it physically fuses with the surrounding bone. If the bone is too thin or soft, this fusion cannot occur, leading to a loose implant and eventual failure. Bone grafting addresses this by providing a scaffold for your body to regenerate new, strong bone, ensuring the long-term stability and success of your dental implant.

When is Bone Grafting Needed?

Not all patients require a bone graft before receiving dental implants. However, your oral surgeon may recommend it if they observe signs of significant bone loss, such as:

  • A long period has passed since a tooth extraction, leading to natural bone resorption.
  • The jawbone has been damaged due to advanced gum disease or a previous infection.
  • You are missing multiple teeth, and the underlying bone structure has deteriorated.
  • A physical examination and diagnostic imaging show insufficient bone width or height for a stable implant.

To accurately diagnose the need for a bone graft, the process begins with a thorough examination. At New England Oral, this includes advanced diagnostic tools like digital X-rays and 3D cone beam CT scans. These detailed images allow our surgeons to precisely measure your jawbone density and volume, pinpointing any areas of deficiency. Whether you need a single implant or a full-mouth restoration, a precise diagnosis is the first step toward a successful outcome.

Types of Bone Grafting Materials

The material used for a bone graft can come from several sources, each with its own advantages. The choice of material often depends on the specific needs of the patient and the extent of the bone loss.

A. Autografts

This is considered the “gold standard” of bone grafting. An autograft uses bone harvested from the patient’s own body, typically from the chin, jaw, or a less common site like the hip.

  • Benefits: It has the highest success rate because the bone is living and contains your own bone cells and growth factors, eliminating the risk of rejection.
  • Drawback: It requires a second surgical site to harvest the bone, which can lead to additional discomfort and a longer recovery time.

B. Allografts

An allograft uses bone from a human donor (cadaver). The bone is carefully screened, processed, and sterilized to ensure safety and prevent disease transmission.

  • Benefits: It eliminates the need for a second surgical site on the patient, reducing surgical time and post-operative discomfort.
  • Drawback: While the risk is minimal due to processing, there is a theoretical risk of rejection, and it does not contain the patient’s own living cells.

C. Xenografts

This type of graft material is derived from an animal source, most commonly bovine (cow) bone. The bone is meticulously processed to remove all organic components, leaving only the mineral matrix.

  • Benefits: It is highly biocompatible and widely available, and it provides an excellent scaffold for your body’s natural bone to grow into.
  • Drawback: It is not as effective at stimulating new bone growth as an autograft.

D. Alloplasts

Alloplasts are synthetic bone materials made from biocompatible substances like calcium phosphate or bioactive glass.

  • Benefits: They offer a completely sterile and disease-free option, as they are not derived from a human or animal source. The composition is also highly controlled, making them predictable.
  • Drawback: They are primarily used as a scaffold and are not as osteoinductive (able to stimulate new bone growth) as autografts.

Bone Grafting Techniques for Implants

The technique used for your bone graft will be tailored to the specific location and severity of your bone loss. Some of the most common techniques used at New England Oral include:

  • Socket Preservation (Ridge Preservation) This technique is performed immediately after a tooth extraction. The graft material is placed directly into the empty socket to prevent the natural collapse of the bone ridge that would otherwise occur. This maintains the bone volume, making future implant placement much simpler.
  • Sinus Lift (Sinus Augmentation) The upper jaw, especially in the back molar and premolar areas, often has less bone density due to the presence of the maxillary sinuses. A sinus lift is a procedure that gently raises the sinus membrane and places bone graft material in the newly created space. This increases the vertical bone height, creating a stable foundation for implants in the upper jaw.
  • Block Bone Grafting Used for more severe bone defects, this technique involves using a solid block of bone, often an autograft, and securing it to the deficient area with small screws. It is an effective method for rebuilding large sections of missing bone.
  • Onlay Grafting This technique is used when the jawbone is too narrow to support an implant. A bone graft is placed directly on top of the existing bone ridge to increase its width, creating enough space for the implant post.
  • Guided Bone Regeneration (GBR) GBR is a versatile technique that uses a barrier membrane to protect the bone graft material. The membrane, which can be absorbable or non-absorbable, prevents faster-growing gum tissue from invading the space where the bone graft is meant to heal, allowing the bone cells time to regenerate undisturbed.

The Bone Grafting Procedure Step-by-Step

While each procedure is customized, the general process for bone grafting is as follows:

  • Pre-surgical Consultation & Imaging: Your oral surgeon will review your 3D CT scans and X-rays to create a precise surgical plan.
  • Anesthesia: The procedure can be performed under local anesthesia, conscious sedation, or general anesthesia, depending on the complexity and your comfort level.
  • Placement of Graft Material: An incision is made in the gum tissue to expose the jawbone. The chosen bone graft material is then carefully placed in the deficient area.
  • Use of Membranes: A protective membrane and/or biologics (like platelet-rich fibrin) may be used to cover the graft, promote healing, and protect the site.
  • Suturing: The gum tissue is gently repositioned and sutured closed.
  • Post-Procedure Care: You will be given detailed instructions on how to care for the surgical site, manage any discomfort, and what to expect during the initial healing phase.

Recovery After Bone Grafting

The recovery process is a critical phase for the success of your bone grafting procedure.

  • Typical Healing Timeline: The full healing and maturation of the graft can take anywhere from 4 to 9 months, with larger or more complex grafts requiring more time. This period allows the graft material to integrate with your existing bone and create a strong, new bone matrix.
  • Immediate Post-Op Expectations: You can expect some swelling, bruising, and mild discomfort in the first few days, which can be managed with prescribed pain medication and ice packs.
  • Diet Modifications: A soft diet is recommended for the first few weeks to avoid putting pressure on the surgical site.
  • Oral Hygiene: You will be instructed on how to carefully clean the area to prevent infection without disturbing the graft.
  • Follow-up Appointments: Regular check-ups with your oral surgeon are essential to monitor the healing process.
  • Implant Placement: Once the bone graft has fully healed and the new bone is strong enough to support an implant, you will be scheduled for the dental implant placement procedure.

Benefits of Bone Grafting for Implants

The benefits of bone grafting extend far beyond simply making implant placement possible. It is a vital step that ensures the long-term success and health of your entire smile.

  • Increases Implant Stability and Longevity: By creating a dense and healthy foundation, bone grafting ensures the implant is securely anchored, significantly increasing its stability and lifespan.
  • Prevents Further Jawbone Deterioration: The graft stimulates new bone growth and prevents the ongoing bone loss that naturally occurs after tooth extraction.
  • Improves Facial Structure and Aesthetics: Bone loss in the jaw can lead to a “caved-in” facial appearance. Rebuilding the bone helps restore the natural contours of your face.
  • Expands Treatment Eligibility: Bone grafting allows patients who would otherwise not be candidates for implants to receive this life-changing treatment.
  • Enhances Chewing Ability and Oral Function: A solid foundation for implants leads to a stronger bite force and a more comfortable, natural chewing experience.

Risks and Considerations

As with any surgical procedure, there are some potential risks associated with bone grafting, though they are rare and carefully managed. These can include infection, graft rejection, or prolonged healing. A key factor in minimizing these risks is the experience of your surgeon. The oral surgeons at New England Oral are specialists in these procedures, with extensive training and expertise in a wide range of grafting techniques. We use the latest technology and sterile protocols to ensure the safest and most predictable outcomes possible.

Bone Grafting Cost Overview

The bone grafting cost can vary widely depending on the complexity of the procedure and the type of material used.

  • Average Cost Ranges:
    • A small, simple graft (e.g., socket preservation) can range from $300 to $800+ CAD.
    • A larger, more complex graft (e.g., sinus lift or block graft) can range from $1,000 to $3,000+ CAD per site.
  • Factors Influencing Cost:
    • Material Type: Autografts are generally more expensive due to the need for a second surgical site, while allografts and xenografts can be more cost-effective.
    • Surgical Complexity: A routine socket preservation is less costly than a complex sinus lift or a block graft.
    • Additional Procedures: The need for other treatments, like guided bone regeneration with a membrane, will add to the total cost.
    • Financing and Insurance: While coverage can be limited, many dental plans may offer some coverage, and financing options can make the cost more manageable. At New England Oral, we work with you to understand your benefits and explore payment solutions.
