Your Initial Appointment at
Our Clinic

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Your surgical referral slip and any x-rays if applicable

A list of medications you are presently taking

Your dental insurance information

IMPORTANT

All patients under the age of 18 years of age should be accompanied by a parent or guardian at the consultation visit.

preoperative consultation

A preoperative consultation and physical examination is mandatory for patients undergoing IV Sedation or General Anaesthesia for surgery.

medical condition

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-rays

If your dentist or physician has taken x-rays you may request that they be forwarded to our office. If there is not enough time for us to receive them before your appointment, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office at the time of your appointment.

Financial Policy

After Dr. Bahi,oral and maxillofacial surgeon in Surrey, examines you and diagnosis what needs to be done our staff will give you an estimate of the cost for the procedure. If you decide to have the procedure done the same day as the consult and you have insurance we can give you an estimate of what your insurance will cover.  If you decide to have your procedure done at a later date we will send Dr. Bahi’s treatment plan to your insurance company to determine your exact coverage for the procedure. At the time service is rendered you will be responsible for paying our office the amount of the cost that is not covered by your insurance, the balance will be billed directly to your insurance company by our office.  If the insurance company does not cover the full amount after we submit the claim, we will invoice you accordingly.  Payment is due upon receipt of the invoice.

We deliver the finest care at the most reasonable cost to our patients.  If you do not have insurance, payment is due in full at the time service is rendered unless other arrangements have been made in advance.  For your convenience, we accept Visa, MasterCard and Interac
